Our aim is to simplify the process for you
In order to provide our services, ART Surgical collaborates with most insurance plans, unless transgender procedures are explicitly excluded. However, we are unable to work with the following:
- Medicare
- Medicaid
- Straight Medi-Cal
Prior to your consultation with Dr. Rodriguez, we will require the following documents: your insurance card, driver’s license or ID, and possibly photographs of the face or chest depending on the surgery. Additionally, please ensure that the intake forms are completed.
Following your personalized consultation with Dr. Angela Rodriguez, we will assist you in navigating the insurance procedure, ensuring that you feel well-prepared. Below is the insurance-related information you should have ready:
- A copy of the front and back of your insurance card.
- Any updates regarding your insurance.
- If you possess multiple insurance policies, kindly inform us about your primary plan. If you are unsure, please contact your insurance companies for clarification.
- If you are enrolled in an HMO plan or in a managed Medi-Cal plan, it is essential to obtain an updated referral from your primary care physician within 90 days of your scheduled surgery date.
